Exhibit A <br />SCOPE OF SERVICES — Baldwin Torres <br />A. Provider shall conduct Martial Arts techniques for children grid adults. <br />B, Karate Do Kiai class will consist of monthly classes, held 2 days per week, 1 hour per day. <br />C. Karate Do Kiai class will consist of monthly classes, held 2 days per week, 1 hour per day, <br />D. Provider and City shall mutually agree upon a schedule for classes, including the location, specific days and <br />hours when class will be held and holidays to be observed, <br />E. Provider will provide and be responsible for equipment, records, and personnel and cleanup of the facilities <br />and materials necessary to ensure the safety and effectiveness of said instruction. <br />CLASS SIZE <br />A, Each class must have a minimum of 4 paid students and no more than a maximum of 50 or as established <br />by mutual agreement of the City and Provider. <br />B. No registration will be accepted after the second meeting of class. <br />C. In the event the minimum number of enrollees is not realized by the second meeting of the class, the class <br />shall be canceled. Provider will be under no obligation to provide services for the cancelled class, and the <br />City will have no further obligations to pay Provider compensation for the remaining classes that were <br />cancelled in that session. <br />CLASS FEES <br />1) Each participant shall pay class registration fees as established by the City. <br />2) Provider may not waive class participation/registration fees. <br />3) Only registered participants may participate in the class. <br />4) City shall prepare class rosters and provide a copy to Provider, <br />5) Any refunds to participants will be made in accordance with City policy, <br />
