Exhibit A <br />SCOPE OF SERVICES — Kay Cotter <br />Provider shall conduct Adult Ceramics -Hand building for children and adults. <br />A. Adult Ceramics/Hand building class will consist of a monthly session, 2 days per week, and 2 hours <br />per day. <br />B. Creative Sculpting with Clay class will consist of a monthly session, 2 days per week, 1'/x hour per <br />day. <br />C. Classes will be throughout the year. <br />D. Provider and City shall mutually agree upon a schedule for classes, including the location, specific days <br />and hours when class will be held and holidays to be observed. <br />E. Provider shall provide materials, supplies, equipment, records and personnel, Provider shall be <br />responsible for clean-up of the facilities and materials and shall ensure the safety and effectiveness of <br />instruction. <br />CLASS SIZE <br />1) Each Adult Ceramics/Hand buildine class must have a minimum of 2 paid students and no more than a <br />maximum of 15 or as established by mutual agreement of the City and Provider. <br />2) No registration will be accepted after the second meeting of class. <br />3) In the event the minimum number of enrollees is not realized by the second meeting of the class, the <br />class shall be cancelled. Provider will be under no obligation to provide services for the cancelled class, <br />and the City will have no further obligation to pay Provider compensation for the remaining classes that <br />were cancelled in that session. <br />CLASSTEES <br />1) Each participant shall pay class registration fees as established by the City, <br />2) Provider may not waive class participation/registration fees. <br />3) Only registered participants may participate in the class. <br />4) Any materials fee shall be established by mutual agreement of the City and Provider and shall be <br />payable directly to Provider. <br />5) Any refunds to participants will be made in accordance with City policy. <br />
