Exhibit A <br />SCOPE OF SERVICES— Maria "RIA" Maddock <br />Provider will. teach Zumba w/Ria and various classes for. age 13 & up. <br />A. Zomba w/Ria- class consists of monthly session, held 2 days per week, 50 minutes per day -$20/per 2 day <br />session. (M & W) or $5.00 drop -in fee <br />B. Zomba Saturday Morning w/Ria class consists of monthly session, held I day per week, 50 minutes per day <br />-$15/per I day session. <br />C. Boodle Zumba w/Ria class consists of monthly session, held 3 days per week, 50 minutes per day -$30/per 3 <br />day session. <br />D, Piloxing w/Ria- class consists of monthly session, held 2 days per week; 50 minutos per day -$20/per 2 day <br />session. (T &Th) or $5.00 drop -in fee <br />E. Stretching, Toning, & Tea Willa class consists of monthly session, held 2 days per week, 30 minutes per day <br />-$12/per 2 day session or $5.00 drop -in fee <br />F. Provider and City staff shall mutually agree upon a schedule for classes, including the location, <br />specific days and hours when class will be held and holidays to be observed. <br />G. Provider shall provide materials, supplies, equipment, records and personnel. Provider shall be <br />responsible for clean-up of the facilities and materials and shall ensure the safety acid effectiveness of <br />instruction. <br />CLASS SIZE <br />A. Each class must have a minimum of 1 paid student and no more than a maximum of 65 students or as <br />established by mutual agreement of City and Provider. <br />B. No registration will be accepted after the second meeting of class. <br />C. In the event the minimum number of enrollees is not realized by the second meeting of the class, the class shall <br />be canceled. Provider will be under no obligation to provide services for the cancelled class, and the City will <br />have no further obligations to pay Provider compensation for the remaining classes that were cancelled in that <br />session. <br />CLASS FEES <br />A. Each participant shall pay a $30/$20/$15/$12, class registration fee per month or $5.00 drop -in fee <br />B. Any refunds to participants will be made in accordance with City policy. <br />C. Provider may not waive class participation/registration fees. <br />
