ARTICLE XI <br />11.0 EMPLOYEE INSURANCE <br />11.1 Health Insurance, The City shall contribute toward medical premiums for each employee <br />covered by this MOU that amount equal to the premium in effect for each tier of the <br />CalPERS Kaiser Permanente HMO Other Southern California plan, as determined by the <br />employee's coverage selection. Any contribution necessary to maintain benefits under <br />said medical plans in excess of the amount set forth above shall be borne entirely by the <br />employee. <br />11.2 Dental Insurance. <br />A. The City shall continue to contribute the current maximum amount of $90 per <br />month per SAMA employee for employee dental insurance premiums. Effective <br />January 1, 2016, the City contribution amount will be increased to a maximum of <br />one hundred ten dollars ($110) per month per employee. <br />B. Any contribution necessary to maintain benefits under said dental plans in excess <br />of the amount set forth above shall be borne entirely by the employee. <br />11.3 Long -Term Disability- &T,D,) Insurance. The City shall continue to pay one hundred <br />percent (100 %) of the premium cost for a 60 -day elimination period long term disability <br />insurance plan for employee covered by this MOU. <br />11A Life Insurance. The City shall continue to pay one hundred percent (100 %) of the premium <br />cost for term life insurance coverage under the policy it maintains on behalf of employees <br />in order to provide employees covered by this MOU with life insurance coverage in an <br />amount equal to twice such employee's annual rate of salary to a maximum of three hundred <br />thousand ($300,000) provided said employees can provide evidence of insurability of <br />coverage, if so required by the terms and conditions of said term life insurance policy. <br />In the event any such employee is determined to be ineligible for said insurance coverage, <br />the City will attempt to provide as much coverage as may be obtained at reasonable cost <br />not to exceed the actual monthly premium amount allotted for said coverage without having <br />to provide evidence of insurability. <br />11.5 The City shall retain the right to change health, dental and life insurance carriers, <br />55394.00000 \13081031.9 <br />37 <br />25H -39 <br />
