Mileage will be charged at a rate of $0.65 per mile. Mileage will not be charged <br />when field truck is used and charged. Overnight stay per diem will be charged $135 per day. <br />Equipment <br />Field Truck and Field Test (Nuclear Gage) Equipment $12/hour (max $90 /day) <br />Field Truck $9/hour (max $70 /day) <br />Vibration & Environmental Monitoring Equipment Separate Schedule <br />Outside Services <br />Rental of special equipment not ordinarily furnished by Hushmand Associates, Inc. and <br />all other costs such as labor, special printing, expendable field supplies, etc. Cost + 15% <br />Terms <br />Billings are payable upon presentation and are past due 30 days from the invoice date. A finance charge <br />of 1.5 percent per month, or the maximum amount allowed by law, will be charged on past -due accounts. <br />25M -457 <br />
