GL4171421 THIS ENDORSEMENT CHANGES THE POLICY. PLEASE READ IT CAREFULLY. ADDITIONAL INSURED - OWNERS, LESSEES OR CONTRACTORS - I SCHEDULED PERSON OR ORGANIZATION This endorsement modifies insurance provided under the following: COMMERCIAL GENERAL LIABILITY COVERAGE PART Name of Additional Insured P,erson(s) or Organization(s,) (Additional Insured): Location(s) of Covered Operations: All persons or organization as required Locations as required by a by a written contract or agreement written contract or agreement with the named insured, with the named insured. All work, including materials, parts or equipment furnished in connection with such work, on the project (other than service, maintenance or repairs) to be per-formed by or on behalf of the additional insured(s) at the location of the covered operations has been completed; or 2.
