Name of Organization <br />Name of Funded Proc <br />City of Santa Ana CDBG Scope of Work <br />Program Year 2015 -16 (July 1, 2015 - June 30, 2016) <br />America On Track <br />Brighter Futures for Children of Prisoners <br />Annual Accomplishment Goal <br />be served during the 12 -month contract period. <br />a Ana Participants 100% 30 Low Income Participants 100% <br />Program and Funding Description <br />Children of prisoners are often the "forgotten" children of our society, and they deserve the chance to discover a <br />brighter future, which is why America On Track (AOT) implements two highly individualized programs for them: <br />Mentoring Children of Prisoners and Brighter Futures for Children of Prisoners. Launched in 2004 they are designed <br />to empower the most vulnerable high -risk youth who tend to live in the most impoverished neighborhoods and are <br />living below the Federal poverty level. The programs provide them with opportunities for life -long success. <br />Our Mentoring Children of Prisoners program promotes the importance of role models, and AOT carefully <br />selects, screens, trains and matches qualified adults with children of prisoners. Their mentor is usually one of the <br />only positive adult role models in their life and represents an anchor in a turbulent sea. Mentors are required to <br />see their mentees 4x per month and provide 8 hours of mentoring per month for a minimum of one year, and also <br />turn in monthly activity notes. The outings take place at a selected site such as a museum, park or library, and vary <br />each week. Data indicates that mentoring programs have reduced first time drug use by almost 50% and first -time <br />alcohol use by 33 %, among other benefits. <br />Our Brighter Futures for Children of Prisoners program compliments the mentoring program by adding year - <br />round creative activities as well as wrap- around services /resources for their families. ACT provides STEM bi- <br />monthly workshops, a "College is For Me Too" over -night summer camp experience, a Holiday Party & gifts, Honda <br />Center outings and age- appropriate books. "College Is For Me Too!" is a unique and proven 3 -day yearly overnight <br />structured summer education camp held at CSUF for our mentees (ages 10 +). It is designed so they can <br />experience a college setting and the joy of learning with inspiring college students who are participating as <br />assistants and presenters of the STEM workshops. AOT's project design for the Brighter Futures for Children of <br />Prisoners program carries out multiple strategies to alleviate the negative effects of incarceration. <br />The above -cited comprehensive approach addresses multiple domains: individual, peer, family, school, <br />community, and societal /environmental, as well as multiple risk factors taken into consideration in the project <br />design. <br />America On Track will serve 30 unduplicated Santa Ana mentees /children of prisoners with these grant funds. <br />Activities will include recruiting and training 30 new mentors to be matched with the mentees and providing 90 <br />family members with wrap- around services. <br />Schedule of Performance <br />Unduplicated Estimated <br />Participants Invoicing <br />Quarter 1: JUL 1 - SEP 30 8 $ 7,500 <br />Quarter 2: OCT 1 - DEC 31 7 $ 7,500 <br />Quarter 3: JAN 1 - MAR 31 8 $ 7,500 <br />Quarter 4: APR 1 - JUN 30 7 $ 7,500 <br />30 $ 30,000 <br />Exhibit A <br />Page 1 of 1 <br />
