City of Santa Ana CDBG Scope of Work <br />Program Year 2015 -16 (July 1, 2015 - June 30, 2016) <br />Name of Organization Council On Aging - Orange County <br />Name of Funded Prograr Benefits Enrollment Center <br />Annual Accomplishment Goal <br />Unduplicated Participants anticipated to be served during the 12 -month contract period. <br />520 TOTAL I I 520 Santa Ana Participants 1 100%1 520 Low Income Participants 100% <br />Program and Funding Description <br />Benefits Enrollment Centers (BEC) provide assistance for Medicare - eligible, low- income seniors and persons with <br />disabilities to identify and apply for benefits programs. Trained staff and volunteer counselors assist clients in <br />applying for up to five programs in a single appointment at one site. The program's primary focus is access to five <br />BEC core benefits. <br />1.Medicare Part D Prescription Drug Extra Help -Pays for part D plan premiums, reduces drug co -pays and <br />eliminates the Donut -hole. <br />2.Medicare Saving Programs -Pays Medicare premiums. <br />3.Medi- Cal(California's Medicaid) - Supplements Medicare by paying deductibles and co -pays. <br />4.Cal Supplemental Nutrition Assistance Program(SNAP)- Provides access to nutritious food and alleviates hunger. <br />5.Low- Income Heating and Electric Utility Assistance Program (LIHEAP)- Access savings to help heat and cool one's <br />home. <br />A. Currently, the BEC services are provided at nine locations throughout Orange County. <br />B. There are four BEC sites in Santa Ana, including our central Call Center which is open Monday through Friday <br />from 8Am to 4pm. Additionally, four appointments are available at the Social Security office once a month in <br />English and Spanish. At the Santa Ana Senior Center there are seven appointments available once a month in <br />English and Mandarin. The Southwest Senior Center offers seven appointments once a month in both Spanish and <br />English. With additional funding, we could expand BECs to offer ten additional appointment opportunities at our <br />four Santa Ana locations. <br />C. The services offered through the Call Center are telephone consultations. Appointments at the Social Security <br />Office, Santa Ana Senior Center and Southwest Senior Center are one -on -one in- person counseling sessions for half <br />to one hour per appointment. <br />D.
