City of Santa Ana CDBG Scope of Work <br />Program Year 2015-16 (July 1, 2015 - June 30, 2016) <br />Name of Organization The Cambodian Family <br />Name of Funded Prograr Plan Ahead Youth Program <br />Annual Accomplishment Goal <br />Unduplicated Participants anticipated to be served duringthe 12 -month contract eriod. <br />90 TOTAL 90 Santa Ana Participants 100% 90 Low Income Participants 100% <br />Program and Funding Description <br />Plan Ahead Youth Program (PA)1 j, providescervicesto 55 of Santa Ana's low-income youthsthrough activitiesthat <br />are organized around our four well -integrated institutes (described below). Our Parent Strategy provides25 <br />parents skillsto better support and become more active in their children's lives. Youth activitiestake place at The <br />Cambodian Family Achievement Center after school, Tuesday through Friday, from 3:30-6 PM, and on some <br />Saturdays from 9-3 PM. Youths attend our program year-round, an average of 6-8 hours per week. Staff are <br />bilingual in Cambodian or Spanish. All activities are lead by a partnership of program staff and community <br />volunteers. <br />Academic Institute: Students develop astrong foundation in STBM subjects, strengthen reading and writing <br />skills, improve overall academic performance, graduate high school, and plan their pathstoward college. PAID <br />gives students accessto our multi -faceted community center equipped with computers, high-speed internet, <br />printing, and a small library to support their learning. Program staff work closely with college students and other <br />community volunteersto provide 1.5 hours of daily tutoring aswell as academic workshops to improve their study <br />skills. <br />Leadership Institute: Through weekly Life Skills classes, we help youths develop leadership and social skills, and <br />improve self-esteem and character strengths. <br />Career Institute: To help youths plan their pathstowards becoming a contributing member of society, we host <br />quarterly career guest speaker presentations and workshops on the benefitsof higher education and its gateway to <br />awider range of different career options. <br />Health Institute: Youths develop a health conscious lifestyle through healthy eating and active living. Students <br />engage in 30 minutes of daily exercise during recreation time and an optional mixed -martial arts class. We offer <br />monthly workshopsto discussthe importance of good nutrition, proper portion sizes and healthy eating. <br />In order to increase the likelihood of the youth's success, parents must strongly be involved in our program.
