What <br />specific activities will be undertaken during the contract period. Please be concise in your response. Only the viewable <br />space will print. <br />2 -1 -1 Orange County (2110C) works to end homelessness by strengthening Orange County's (OC) Continuum of <br />Care (CoC) through the use of HMIS so that service providers can most efficiently and effectively assist people in <br />need. Through the proposed FY 2015 -16 program, 2110C will facilitate HMIS coordination, communication, <br />training, and technical assistance of Santa Ana ESG sub - recipients to ensure adherence to data quality, supporting <br />the city's efforts to fully comply with the HEARTH Act of 2009 and address the need for development and <br />implementation of a CA /CI program. 2110C expects all agencies, including ESG HMIS service providers, to have <br />the tools necessary to enter and share quality data following HUD standards, ensuring Santa Ana is well positioned <br />to receive continued HUD funding. <br />fully comply with the HEARTH Act of 2009 and address the need for development and implementation of a CA/CI <br />program <br />Schedule of Performance /Goal Outcomes <br />Estimate the number of ONLY unduplicated Santa Ana participants to be served by the funded program during the 12- <br />month contract period per quarter.
