What specific activities will be <br />undertaken during the contract period. Please be concise in your response. Only the viewable space will print. <br />Illumination Foundation (IF) will use ESG funds to provide social and medical street outreach for the Santa Ana <br />homeless population, primarily the homeless in the Civic Center area. The primary goal for social and medical <br />outreach services in this area will be to identify persons in need of housing services and connect them to our rapid re- <br />housing and homeless prevention services. We will also provide medical outreach via a mobile medical van that will <br />assist homeless residents with vital medical services. <br />The street outreach team will interface with the homeless community through weekly informal walkthroughs at the <br />Civic Center, planned during high congregating hours, such as meal times. The outreach team will share information <br />on services that IF provides, such as housing opportunities, basic needs care (e.g. clothing, hygiene, medical care, <br />behavioral health), and case management services available at the Ross Street Resource Center. The outreach team <br />will also identify families who live on the street, but still have some income, and connect them to rapid re -housing. <br />Case managers and social workers will also be available to meet with clients in order to determine how best to serve <br />and connect them to social services. <br />The IF medical outreach team will provide monthly medical services such as general health checks and consultations, <br />health education, blood pressure checks, glucose screenings, medications, and medical connections/referrals. <br />Services will be provided by our Physician Assistant, along with Kaiser Permanente residents. These medical <br />outreach services are important strategies to reduce and prevent serious medical conditions, as well as provide <br />quality care to the homeless and low-income population in Santa Ana. <br />Schedule of Performance/Goal Outcomes <br />Estimate the number of ONLY unduplicated Santa Ana participants to be served by the funded program during the 12 - <br />month contract period per quarter.
