Please be concise in your response. Only the <br />viewable space will print. <br />Intake and Assessment: Case managers conduct phone interviews and determine eligibility for our programs <br />and identify families that would benefit from our Rapid Re- Housing Initiative. Housing Search and <br />Placement: Our Re- Housing Case Manager will conduct an initial interview to determine their housing <br />needs, including appropriate size, price range, and location. <br />Supportive Services: Thomas House will pay housing start -up costs including security deposits, first and last <br />month's rent, and utility deposits. The following additional supportive services will be available for up to <br />twelve months: <br />Declining cost -based rent subsidies: An example of the declining cost based rent subsidy is as follows: <br />Thomas House will pay the full cost of rent in the 1st month, 80% in the 2nd month, 60% in the 3rd month, <br />and 40% in the 4th — 6th months as needed. <br />Estimate the number of ONLY unduplicated Santa Ana participants to be served by the funded program during the 12- <br />month contract period per quarter. (Enter number of new Santa Ana Participants served each quarter.
