ARTICLE XI <br />11,0 EMPLOYEE INSURANCE <br />11.1 Health Insurance. The City shall contribute toward the payment of premiums for affected <br />employees and their dependents under the existing health insurance programs or new <br />program or programs providing substantially similar or improved coverage and benefits <br />selected in accordance with procedures in effect on the effective date of this MOU: <br />A. Effective January 1, 2016, the City's contributions for medical insurance will be <br />increased and indexed to the Kaiser Permanente HMO Other Southern California <br />plan, as determined by the employees' coverage selection available through <br />CalPERS. <br />B. Any contribution necessary to maintain benefits under said medical plans in <br />excess of the wnoimt set forth above shall be borne entirely by the employee. <br />C. An employee who is covered tinder a spouse's non -City sponsored health plan <br />and voluntarily waives, in writing, their City paid health insurance coverage will <br />receive a cash payment each month in an amount equal to 50% of the total <br />monthly premium amount for the City's lowest "employee -only" coverage. <br />11.2 Dental Insurance. The City shall contribute a maximtun amount of tip to ninety ($90) <br />dollars per month per employee toward the payment of premiums for dental insurance <br />plans provided by the City for employees covered by this MOU and their eligible <br />dependents.. Any contribution necessary to maintain benefits under said dental plans in <br />excess of the amount set forth above shall be borne entirely by the employee, <br />11.3 Long -Term Disability (LTD) Insurauce. The City shall contribute up to ten dollars and <br />thirty -nine cents ($10.39) per month to employees enrolled in the 130 day elimination <br />period LTD plan. For employees who elect to upgrade their LTD benefit coverage to a <br />60 day elimination period plan, the City will contribute up to eleven dollars ($11.00) per <br />month. Any amount necessary to maintain benefits under the long tern disability <br />insurance plans provided by the City in excess of the amounts set forth above shall be <br />borne by the employee. <br />11.4 Life Insurance, The City shall provide its employees with a twenty thousand dollar <br />($20,000) term life insurance policy and a twenty thousand dollar ($20,000) Accidental <br />Death and Dismemberment (AD &D) coverage at no cost to the employee, <br />11.5 When there is a need to discuss matters relating to employee insurance and the City <br />believes it would be beneficial to involve an Insurance Committee, file Union shall have <br />an equal number of representatives as the City on such a committee to meet as necessary. <br />49 <br />25A -53 <br />
