Exhibit A <br />Delhi Center ADA Upgrades <br />Scope /Statement of Work <br />Delhi Center's efforts under this Agreement will be to provide public facility improvements for the <br />removal of barriers to accessibility at Delhi Center to comply with the American with Disabilities Act <br />(ADA). Funding of this Agreement is to support project management, design, engineering, bid <br />preparation, labor compliance, and installation and construction costs. <br />An assessment will be conducted to determine priority of ADA improvements which will include but is <br />not limited to 18 sets of doors as listed in Exhibit A -1. Once the assessment is completed, a priority list <br />of improvements will be submitted to the City for approval from the CDBG project manager. <br />Major tasks that the Subrecipient will perform include, but are not limited to, the following: <br />• ADA assessment to prioritize improvements <br />• Bid Preparation to include labor standards language and appropriate wage decisions <br />• Solicitation and selection of contractors <br />• Labor Compliance Enforcement <br />• Verification that contractor expenses are reasonable and work has been completed properly <br />• Approval of contractor payments <br />• Maintenance of project file to include: <br />• Professional services procurement <br />• Bid document <br />• Executed Contracts /Bid Documents <br />• Board Minutes for Approval of Contracts or Bids <br />• Review and Certification of Insurance Coverage /Bonding <br />• Certified Construction Payroll Records (Davis -Bacon applicable) <br />• Anti - discrimination, Fair Housing, EEO, ADA /504 Certifications <br />• Procurement, Bonding, Insurance <br />
