ARTICLE XI <br />11.0 EMPLOYEE INSURANCE <br />11.1 Flealth lnstiranee, The City shall contribute toward the payment of premhtms for affected <br />employees and their dependents under the existing health insurance programs or new <br />Program or programs providing substantially similar or improved coverage and bonefits <br />selected in accordance with procedures in effect on the effective date of this MOU: <br />A. Effective January 1, 2016, the City's contributions for medical insurance wilt be <br />inoroased and indexed to the Kaiser Permanente IIMO Other Souther California <br />plan, as dotorininod by the employees' coverage selection available through <br />CaIPERS. <br />S. Any contribution necossary to maintain benefits under said medical plans in <br />excess of the amount sot forth above shaft be borne entirely by the employee. <br />C. An employee who is covered tmdor a spouse's non -City sponsored health plan <br />and voluntarily waives, in writing, their City paid health insurance coverage will <br />receive a cash payment each month in an amoturt equal to 50% of the total <br />monthly premium amount for the City, s lowest "employee- only" coverage. <br />11.2 Dental Insurance, The City shall contribute a maximum amount of up to ninety ($90) <br />dollars per month per employee toward the payment of premiums for dental insurance <br />plans provided by the City for employees covered by this MOIJ and their eligible <br />dependents.. Any contribution necossary to maintain benefits under said dental plans in <br />excess of the amount set forth above shall be borne entirely by the employee, <br />11.3 Long -Tort Disability (LTDI Insurance. The City shall contribute up to ten dollars and <br />thirty -nhac cents ($10,39) per month to employees enrolled in the 130 day elimination <br />period LTD plan. For employeos who elect to upgrade their LTD benefit coverage to a <br />60 day elimination period plan, the City will contribute up to eleven dollars ($11,00) per <br />month. Any amount necessary to maintain benefits under the long term disability <br />insurance plans provided by the City in excess of the amounts set forth above shall be <br />borne by the employee. <br />11.4 Life Insurane , The City shall provide its employees with a twenty thousand dollar <br />($20,000) term life insurance policy and a twenty thousand dollar ($20,000) Accidental <br />Death and Dismemberment (AD &D) coverage at no cost to the employee. <br />11.5 When there is a need to discuss matters relating to employee insurance and the City <br />believes it would be beneficial to involve an Insurance Committee, the Union shalt have <br />an equal number of representatives as the City on such a committee to meet as necessary. <br />49 <br />25A -53 <br />
