STATEMENT OF QUALIFICATIONS <br /> <br /> <br />18 | Page <br /> <br /> <br />SERVICE PROVIDER EXPERIENCE <br />A narrative describing recent experience, with an <br />emphasis on experience gained in the last five years <br />on projects similar to the one being proposed. <br />a. Include project name and type (homeless, special <br />needs, senior, large family, etc.), project address, unit <br />count and bedroom type, affordability requirements, <br />and funding sources in your description. <br />b. Include a description of current involvement with <br />the Orange County Continuum of Care and the <br />Orange County Coordinated Entry system. (If from <br />another county, may use that jurisdiction’s CoC.) <br />Overview / History <br />Mercy House has operated successful homeless <br />service programs for over 25 years and in that time <br />has served over 70,000 individuals. This includes <br />operating a variety of programs ranging from <br />homeless prevention, emergency services and <br />shelters, transitional housing, aftercare programs, <br />rapid re-housing programs as well as permanent <br />supportive housing facilities and residential services <br />program. The diversity of these programs has <br />afforded us the experience of working with a wide <br />variety of homeless subpopulations including, but <br />not limited to; families, adult men and women, <br />mothers and their children, Veterans, seniors, <br />chronically homeless individuals, persons living <br />with HIV/AIDS, individuals overcoming substance <br />addictions, and those who are both physically <br />and/or mentally disabled. <br /> <br />Mercy House is uniquely qualified to provide <br />permanent supportive housing in the City of Santa <br />Ana. Our history of operations within the city over <br />the past 25 years affords us extensive knowledge <br />and experience in regards to the needs and desires <br />of residents, neighborhood associations, and the <br />clients we serve. Santa Ana has served as Mercy <br />House’s center of operations as well as the site of <br />many of our programs and homes. We have <br />operated several programs within Santa Ana <br />including transitional housing for single men, single <br />women with children, single men and women with <br />HIV and AIDS, the Armory Emergency Shelter <br />program as well as Rapid Re-Housing and <br />Homelessness Prevention programs that directly <br />benefit the residents of Santa Ana. Mercy House <br />has been recognized by City and County officials <br />for their service to the homeless in the community. <br />Most recently, Father Jerome and Executive <br />Director, Larry Haynes were recognized in <br />December, 2015 by Mayor Miguel Pulido for their <br />work to end homelessness in the City of Santa <br />Ana. <br />Key Accomplishments: 2014-2015 Fiscal Year <br />HOUSING PLACEMENT <br />During the 2014-2015 fiscal year, Mercy House <br />set a goal to prevent or end the homelessness of <br />500 people. Exceeding expectations, we were able <br />to secure and maintain stable housing for 816 <br />individuals who came through our doors! <br /> <br />HOMELESS PREVENTION <br />Mercy House’s Homeless Prevention Program <br />rescued 9 families, including 17 men, women and <br />children, from losing their homes, falling into <br />homelessness and entering the shelter system. <br /> <br />3-28
