STATEMENT OF QUALIFICATIONS <br /> <br /> <br />19 | Page <br /> <br /> <br />EMERGENCY SHELTERS AND SEVICES <br />Mercy House served 4,852 homeless individuals; <br />meeting immediate needs and providing <br />connections to restore dignity and stability. This <br />includes 2,078 people through our Emergency <br />Shelter Programs, 536 through our Family <br />Redirection Program, 1,255 at the Ontario Access <br />Center, and 340 at the Anaheim Check in Center. <br /> <br />TRANSITIONAL SHELTER <br />Mercy House served 218 homeless men, women <br />and children in their transitional shelters; of <br />those 67% successfully transitioned into their own <br />stable housing, or remain active in our programs. <br /> <br />PERMANENT HOUSING <br />In 2014-2015, Mercy House’s ongoing permanent <br />housing programs were able to provide housing to <br />816 unduplicated individuals; including 431 people <br />through Rapid Re-Housing, 42 through our <br />Shelter Plus Care program, 86 in our Master <br />Leasing Program, and 106 in our affordable <br />housing programs. <br /> <br />SERVICE PROVIDER <br />PROJECT EXPERIENCE <br />Mercy House has extensive experience in <br />providing supportive services similar to those <br />proposed. Our primary focus as an agency is <br />providing housing placement and housing <br />stabilization services to homeless and chronically <br />homeless individuals and families. <br /> <br />Staff is trained in Critical Time Intervention (CTI) <br />model of early engagement which provides <br />emotional and practical support during the critical <br />time of transition from street to home and <br />strengthen each client’s ties to services, family and <br />friends. <br /> <br />Supportive services focus on intensive case <br />management to ensure that every client has the <br />chance to succeed and remain in permanent, <br />stable housing. Intensive case management <br />includes individualized service plans designed to <br />identify and address their individual needs and <br />barriers to maintaining permanent housing. <br />Housing Stabilization Case Managers work to <br />create and achieve goals, coordinate services, <br />counseling and support, crisis intervention, <br />intensive assistance with accessing mainstream <br />benefits and other community-based resources. <br /> <br />Permanent Supportive Housing Programs <br />Project Type: Permanent Supportive Housing <br />Population Served: Chronically homeless <br />individuals and families <br />Project Address: Scattered-site apartments <br />throughout Orange County <br />Unit Count/Bedroom Type: 140 units <br />Affordability Requirements: No income limits or <br />requirements <br />Funding Sources: United States Department of <br />Housing and Urban Development, private sources <br />Service Start Date: 2013 <br /> <br />San Miguel <br />Project Type: Permanent housing with supportive <br />services <br />3-29
