�J <br />A- 2015- 086 -01 <br />• 1M] __rel - <br />This First Amendment to Subgrant (hereinafter referenced as "Amendment ") is dated <br />effective as of the date fully executed by all parties ( "Effective Date" j and is entered into <br />between Community Action Partnership of Orange County, a California non - profit <br />corporation, (hereinafter referenced as "Subgrantor ") and City of Santa Ana, a municipal <br />corporation (hereinafter referenced as "Subgrantee'.') for the funding of Orange County <br />Partnerships to Improve Community Health Grant (hereinafter referenced as "Grant "). <br />Subgrantor and Subgrantee entered into that certain Subgrant dated October 1, 2014. <br />Subgrantor and Subgrantee now desire to amend the terms of the Agreement, as more <br />particularly set forth below: <br />Section 1, Paragraph 1, Exhibit A will be replaced with the Exhibit A attached <br />hereto and made part of the Agreement. <br />2, Section 1, Paragraph 4 is hereby added to the Agreement and shall hereafter be <br />and read as follows: "Grant No, for the first option term is 5 NU58DP005861-02- <br />00 and Subgrantee must document the Grant No. on all communication and <br />correspondence ". <br />3. Section 2, a. will be amended to read as follows: "Term of this Agreement is now <br />extended from Sept 30, 2015 to Sept 29, 2016. ". <br />4. Section 2, b. will be amended to read as follows: "There is one (1) 1 year option <br />remaining and renewal of said option is contingent on adequate funding from the <br />Centers for Disease Control and Prevention (CDC) and satisfactory <br />programmatic progress of Subgrantee. Subgrantee must give ninety (90) day <br />notice in writing as prescribed herein to Subgrantor to exercise the option. ". <br />Section 2, c, will be amended to read as follows: "The maximum amount of <br />funding from September 29, 2015 to Sept 29, 2016 is $119,800; and ". <br />6. Section 3, a. i.
