City of Santa Ana <br />Scope, of Work 9/30/2015 to 9/29/2016 <br />1. City of Santa Ana - institution. <br />2. The City of Santa Ana was sole sourced. City of Santa Ana is the municipal entity responsible for <br />land use decisions; zoning, streets, bike lanes, sidewalks, open space such as parks and trails. The city <br />is responsible for transportation decisions that affect safety such a speed limits, stop signs /lights, <br />lighting. The city provides tap water in public spaces; drinking fountains and hydration stations. <br />3. The contract for this project is September 30, 2015 through September 29, 2016. <br />4. Scope of Work <br />Project Period Objective 02 — Increase the number of people with improved access to environments <br />with healthy food and beverage options. <br />• Support other coalition partners, as needed; Deliverable- Progress /Program Report; and <br />• Encourage resident to consume tap water by supporting a tap water/ rethink your drink campaign. <br />Deliverable- Program /Progress Report; <br />Period Project Objective 03 — Increase the number of people with improved access to physical <br />activity opportunities. <br />• Provide and encourage physical activity opportunities for residents. Deliverable - <br />Progress/Program Report; <br />• Support safe, healthy environments for all individuals that work, play and live in Santa Ana. <br />Deliverable- Progress/Program Report; <br />• Install sixteen bike racks around the city. Deliverable- Installation of bike racks; <br />• Engineer designed intersection enhancements to ensure ADA compliance. Deliverable- design of <br />plan <br />• Install five intersection enhancements to encourage safe and attractive physical activity and to <br />ensure ADA compliance. Deliverable- Installation of enhancements; and <br />• Support and assist, as needed, UCI evaluation efforts. <br />Period Project Objective 05 — Increase the number of public and partner messages promoting <br />healthy lifestyles. <br />• Provide technical assistance to the OC PICH coalition for the development of a social media <br />campaigns, as well as, other mixed communication methods. Deliverable- Attendance and <br />participation in meetings related to OC PIC'H communication /media goals. <br />5. The City's program progress and performance will be monitored by the Program Manager. The <br />Contract Manager will ensure The City's is contract compliant. The following will be required of <br />subcontractors; <br />• Required attendance to monthly OC -PICH planning meetings; <br />• Provide a monthly activity /event calendar to the Program Manager.
