City of Santa Ana CDBG Scope of Work <br />Program Year 2016 -17 (July 1, 2016 - June 30, 2017) <br />Name of Organization Community SerniorSery <br />Name of Funded Prograr Home Delivered and Congregate Meals Program <br />Annual Accomplishment Goal <br />contract period. <br />8751 Low Income Participants 100% <br />Program and Funding Description <br />Funds will be utilized to help fund the raw food costs of meals and wheels and congregate lunch meals to Santa Ana <br />older adult participants. <br />Home Delivered Meal Progam: <br />Participants in the Home Delivered Meals program will receive nutritious meals delivered to their home and case <br />management services. Meals will be delivered by paid and volunteer drivers. Eligibility and case management <br />services will be provided by CSS Case Managers.
