Number of ONLY unduplicated Santa Ana Participants to be served by the funded program, named above, <br />during the 12 -month contract period. <br />1,443 Participants <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12 -month contract <br />period. What specific activities will be undertaken during the contract period. Please be concise in your response. <br />Only the viewable space will print. <br />2110C will faciliate HMIS coordination, communication, training, and technical assistance of Santa Ana ESG <br />sub -recipients to ensure adherence to data quality, supporting the City's efforts to fully comply with the <br />HEARTH Act of 2009 and continued implementation of CES. 2110C will: <br />-provide user meetings, <br />-perform site visits, <br />-provide training/technical assistance, <br />-run data quality reports to post and make available online (www.21loc.org) <br />-complete project set-ups, and <br />Schedule of Performance <br />Estimate the number of ONLY unduplicated Santa Ana articipants to be served by the funded program during the <br />Quarter 1: July 1 - September 30 Al <br />Participants <br />Quarter 2: October 1 - December 31Participants <br />Quarter 3: January 1 - March 31 Participants <br />Quarter 4: April 1 -June 30 Participants <br />Total unduplicated Santa Ana Participants to be served <br />Schedule of Invoicing <br />Estimate the amount of grant funds to be requested during the 12 -month contract period on a quarterly basis. <br />Quarter 1: July 1 - September 30 MOOO.00 <br />0 <br />Quarter 2: October 1 - December 31 Quarter 3: January 1 - March 31 Quarter 4: April 1 -June 30 Total Grant <br />Confirm that the quarterly request add up to grant amount <br />Exhibit A <br />*There are approximately 15, 290 homeless people in OC during the course of the year per the PIT Count &Survey report of 2015 commissioned <br />by 2110C. However, data on the homeless population Is restricted to those agencies whose programs use HMIS. Also, due to changes in program <br />funding from HUD, total number of clients in HMIS maybe impacted, most likely decrease. The goal is to continue capturing as close to 100% of <br />homeless clients in HMIS. <br />
