Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />ity Net will provide street outreach services to 400 homeless neighbors in Santa Ana. Street <br />utreach is service delivery for the specific purpose of reaching out to unsheltered homeless <br />eighbors; connecting them with emergency shelter, housing, or critical services; and providing <br />rgent, non -facility -based care. <br />funds will be used for homeless neighbors in all areas of Santa Ana, with the exception of the <br />Center, which will be served by City Net through a separate contract. <br />Net's street outreach services include services in the following categories: engagement services, <br />management, emergency health, emergency mental health, transportation and services for <br />ecial populations. <br />!se activities are intended to help homeless neighbors to obtain appropriate supportive services, <br />uding assistance in obtaining permanent housing, medical health treatment, mental health <br />atment, counseling, supervision, and other services essential for achieving independent living; <br />ising stability case management; and other Federal, State, local, or private assistance available to <br />ist the program participant in obtaining housing stability. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12 -month <br />contract period per quarter.
