Description of Work - In the space below, describe the program to be funded during the 12 -month contract <br />period. What specific activities will be undertaken during the contract period. Only the viewable space will print. <br />Illumination Foundation will screen and assess walk-in clients (Anaheim and Santa Ana), and will also <br />collaborate with 211 social services community hotline for leveraged marketing and assessment support. <br />Financial literacy and budget management are typically the highest need supports of households that have <br />received homeless prevention assistance. Case Managers provide connection to workforce supports, trade <br />certification, and continuing education. 100% of our current client census are recipients of assistance from <br />public or private agencies that address housing, health, social services, employment, education, or youth <br />needs.
