Please be concise in your <br />response. Only the viewable space will print. <br />Outreach staff guides the client upon intake to Case Management staff. After further assessment, Case <br />Managers establish a plan with Housing Development. Using Critical Time Intervention, Case Managers <br />establish connections between the individual or family to resources in the community where they live that will <br />provide long-term supports for housing stability. CTI is employed in 3 phases, each lasting 3 mos. Upon <br />completion of CTI, clients receive Full Service Partnership or InHome Support Service. Expanded utilization of <br />IF Emergency and Transitional Housing for families, and individuals from the IF Recuperative Care program <br />means there are limited options for ER and transitional housing for local individuals.
