City of Santa Ann Response to Request for Proposal (RFP) Medicnl Services Review 1 2/9/15 <br />Occu-Med's project manager for at least the first year of this <br />contract will be Sean Edin, PTA (Pictured right), <br />Mr. Edin is a certified physical therapist assistant and has <br />performed hundreds of job analysis sessions and thousands <br />of evaluations for Occu-Med clients throughout the United <br />States. These aspects of his job allow him to see many <br />differentjob classifications enhancing his consulting <br />abilities, and coordination in medical appropriateness <br />with Dr. Kaji and legally defensibility with dim Johnson, J.D. <br />(Vlore iilformation regarding Mr, Johnson can be found on <br />Page 3 gfour Proposal). Whether the job <br />class is a Librarian or Police Officer, Mr. Edin is able <br />1 to synthesize the critical data in ajob in order to ensure <br />} quality and continuity in our client employer's occupational <br />medicine Program. Mr. Edin currently oversees multiple <br />Other OCCLI-Med clients throughout California i'oeusing on <br />1' Cities, Counties and Special Districts. <br />! It. Scope of Services (Technical Proposal) <br />1 <br />1 Occu-Med contracts with multiple medical providers throughout the Orange County area which will <br />` allow the City's applicants and employees to always attend an appointment close to his or her currentjob <br />l or home. This service extends beyond the City's immediate geographical area, as Occu-Med has over <br />5,000 medical providers nationwide, so no matter where the City is recruiting from rest-assurecl <br />Occu-Med will have a clinic nearby (qualifications enclosed as part or Appendix A).
