Exhibit 1 <br />COUNTY OF ORANGE CHILD SUPPORT ENFORCEMENT PROVISION <br />CERTIFICATION REQUIREMENTS <br />In order to comply with child support enforcement requirements of the County of Orange, within <br />ten (10) days of award of CONTRACT, the successful CONTRACTOR must furnish to the <br />CONTRACT Administrator, Purchasing Agent or the agency/department Deputy Purchasing <br />Agent: <br />1. In the case of an individual CONTRACTOR, his/her name, date of birth, Social Security <br />number, and residence address; <br />2. In the case of a CONTRACTOR doing business in a form other than as an individual, the <br />name, date of birth, Social Security number, and residence address of each individual who <br />owns an interest of 10 percent or more in the contracting entity; A certification that the <br />CONTRACTOR has fully complied with all applicable federal and state reporting <br />requirements regarding its employees; and <br />3. A certification that the CONTRACTOR has fully complied with all lawfully served Wage and <br />Earnings Assignment Orders and Notices of Assignment and will continue to so comply. <br />The certifications will be stated as follows: <br />"1 certify that City of Santa Ana is in full compliance with all applicable federal and state <br />reporting requirements regarding its employees and with all lawfully served Wage and Earnings <br />Assignment Orders and Notices of Assignments and will continue to be in compliance <br />throughout the term of CONTRACT # 16-28-0039-RWS-P with the County of <br />Orange. I understand that failure to comply shall constitute a material breach of the <br />CONTRACT and that failure to cure such breach within ten (10) calendar days of notice from <br />the COUNTY shall constitute grounds for termination of the CONTRACT. <br />It is expressly understood that this data will be transmitted to governmental agencies charged <br />with the establishment and enforcement of child support orders and for no other purposes and <br />will be held confidential by those agencies. <br />Failure of the CONTRACTOR to timely submit the data and/or certifications required above or to <br />comply with all federal and state reporting requirements for child support enforcement or to <br />comply with all lawfully served Wage and Earnings Assignment Orders and Notices of <br />Assignment shall constitute a material breach of the CONTRACT.
