CITY -OF SANTA -ANA <br />RFP NO.: 16-125 <br />OC STREETCAR DESIGN REVIEW SUPPORT <br />3. DBE subcontract agreements <br />The consultant must submit to the authority copies of executed subcontracts and/or purchase <br />orders (po) for all DBE firms participating on the contract within ten (10) working days of <br />award, The consultant must immediately notify the authority in writing of any problems it <br />may have in obtaining the subcontract agreements from listed DBE firms within the specified <br />time, <br />4. "monthly DBE trucking verification" form <br />Prior to the 10th of each month, the consultant must submit documentation on the "monthly <br />DBE trucking verification" form to the authority showing the amount paid to DBE trucking <br />companies. The consultant must also obtain and submit documentation to the authority <br />showing the amount paid by DBE tracking companies to all firms, including owner -operators, <br />for the leasing of trucks. If the DBE leases trucks from a non -DBE, the contactor may count <br />only the fee or coumnission the DBE receives as a result of the lease arrangement. <br />The consultant must also obtain and submit documentation to the authority showing the truck <br />number, owner's name, California highway patrol ca number, and if applicable, the DBE <br />certification number of the owner of the track for all trucks used during that month. <br />5. "final report -utilization of disadvantaged business enterprises (DBE), first tier <br />subconsultants" <br />Upon completion of the contract, a summary of these records must be prepared on the: "final <br />report -utilization of disadvantaged business enterprises (DBE), first tier subconsultants" and <br />certified correct by the consultant or the consultant's authorized representative, and must be <br />furnished to the engineer. The form must be furnished to the authority within ninety (90) days <br />from the date of contract acceptance. The amount of $10,000 will be withheld from payment <br />until a satisfactory form is submitted, <br />6. "disadvantaged business enterprises (DBE) certification status change" <br />If a DBE sub is decertified during the life of the project, the decertified subconsultant must <br />notify the consultant in writing with the date of decertification. If a subconsultant becomes a <br />certified DBE during the life of the project, the subconsultant must notify the consultant in <br />writing with the date of certification (attach DBE certification/decertification letter). The <br />consultant must furnish the written documentation to the authority. <br />Upon completion of the contract, the "disadvantaged business enterprises (DBE) certification <br />status change" must be signed and certified correct by the consultant indicating the DBEs' <br />existing certification status. If there are no changes, please indicate "no changes". The <br />certified form must be furnished to the authority within ninety (90) days from the date of <br />contract acceptance. <br />V.
