The following is the Work Plan that summarizes our proposed services: <br />Mme and Sub -Contractor Verification <br />Prior to the pre -construction meeting, CHS will verify that the Prime <br />Contractor and any sub -contractors hold current State of California <br />contractor's licenses and/or certifications, and have not been debarred by <br />the Federal Department of Labor from performing work on publicly -funded <br />construction projects. <br />Pre -construction Meeting <br />CHS shall attend the preconstruction meeting. CHS will document the Prime <br />Contractor attendance at the pre -construction meeting and shall place a <br />copy of the meeting minutes in the Project Labor Standards enforcement file. <br />Such minutes shall also be provided to all attendees of the pre -construction <br />meeting. <br />At the pre -construction meeting, CHS will instruct the attendees concerning <br />the Federal regulations that must be met. During the pre -construction <br />meeting and during the completion of the construction activities, CHS will <br />answer all questions concerning prevailing wage compliance and Section 3 <br />procedures, if any. CHS will provide Section 3 documentation, when <br />required. <br />CHS will provide the attendees with the following documents: <br />s Attendance Sheet <br />Wage Posters <br />Certification for Applicable Fringe Benefit Payment <br />Certificate of Understanding and Authorization <br />a Contractor Information Sheet <br />Employee Information Sheet <br />Minutes of the pre -construction meeting <br />a Federal Payroll Form <br />Federal Statement of Compliance <br />Section 3 requirements <br />13 <br />
