CITY OF SAN`Cr# ri,NA <br />RFP NOs; 1:6.090 <br />WATER WELL, PUMP AND MOTOR RE.PA.IR SERVICES <br />Contractor and or Contractor's subcantraotors must possess and maintain the following active and valid <br />icenses as applicable to individual subcontractor <br />1. State of California Water Well Drilling Contractor's C57 License or Contractor's "A" License, <br />2. State of California l lmited Specialty Contractor's 0-61. su.bcateggry N1aghIr1pry .and Pump <br />Contractor's D-21 License. <br />3. Have a minimum of five years of experience performing groundwater wefts and pump pulling <br />services. If a Subcontractor is used, Subcontractor must also have a minimum of five years of <br />experience performing services as stated above. Also,, the Contractor's project manager(s), and <br />Subconfraotor's project manager(s), if any, must have a minimum of five years of experience <br />performing groundwater wells, pump pulling services; video inspections and/or motor repairs in <br />addition, the Contractor's project manager(s) and Subcontractor's project manager(s), if any, must <br />have the following certifications: <br />4. State of California Division of Occupational Safety and Health Administration Hazardous Waste <br />Operations Emergency Response (HAZWCPER) Certification <br />5, National Ground WaterAssociation(NGWA)Certification - Pump Installation <br />6, Electrical Apparatus Service Association (EASA) Accredited Member Status <br />Provide all labor, equipment, materials, tools, and supervision "required to perform. as -needed pump <br />pulling and machines repair services, All material, parts, and equipment shall be new, high grade, and <br />free from defects. Such products shall conform to all Federal, State, and local regulations as they <br />relate to waie, r wells services. <br />Obtain all necessary City, County, State, and Federal penbits and/orlicenses for Contractor's <br />activities at no cost, to the Public Works. Agency, <br />Immediately notify .Public Works of any changes in. the ,Contractor's insurance, .permits, and <br />licenses, Failure to do so may result in .the immediate termination of this Contract for default. <br />Utilize protective clothing and equipment as required by CaVOSHA and/or other regulatory agencies. <br />M. Prevailing Wage <br />The Director of the Department of Industrial Relations of the State of California has ascertained the <br />prevailing rate of per diem wages in dollars, based an a working day of eight hours, for each craft. or <br />type of worker Of mechanic needed to execute any construction: or maintenance contract, which may <br />be awarded by Public Works. The current prevailing wage rates as adopted by the Director of the <br />Department of Industrial Relations are incorporated herein by reference and may be accessed at <br />h ttp:llwww. d i r. ca. go v1. <br />The Contractor Is required to pay the prevailing wage rate referred to above and is responsible for <br />selecting the classification of workers which will be required to perform this service in accordance <br />with the Contractor's method of performing the work, Pursuant to Section 1775 of the Labor Code <br />(State of California) Contractor shall forfeit $50 for each calendar day, or portion thereof, for each <br />worker paid, less than the stipulated prevailing wage rates for any public work done under this Contract <br />or by any subcontractor. <br />
