Compute and Enter Extended Unit Price Subtotals & Combined Total (6B). <br />The Work is to be done in strict conformity with the Agreement BeewReRts(as defined in Attachment 2, as <br />modified by the Bank) f8F the feliew Rg sum amounts reflected in Attachments 6A and 6B. <br />Attach — Completed Mandatory ATTACHMENTS 6A & 6B showing Proposer's Cost /Bid <br />Proposal Unit Pricing by AFP Service Code together with Extended Unit Price <br />Subtotals & Combined Total based on City of Santa Ana Bank Analysis Statistics.* <br />[Download, Complete and Return City of Santa Ana Annual Bank Analysis Statistics <br />(12 Month Totals Ending July 2016) Spreadsheet.] <br />* Mandatory Attachments 6A & 6B may be combined with a Proposer's supplemental <br />Cost /Bid Proposal Statement addressing variable pricing factors and terms for <br />requested or proposed ancillary services, etc. <br />If awarded the Agreement, the undersigned agrees to sign a contract that incorporates the <br />documents that comprise said Agreement(as defined in Attachment 2, as modified by the Bank) <br />and f^.n;^ti the ety bORdg RRd insurance certificates within thirty (30) days after <br />receipt of the notice of award of Agreement, and to begin work when directed by the City. <br />It is understood that this Cost /Bid Proposal is based upon completion of work commencing on the <br />date set forth in the Agreement issued by the City to the Proposer. <br />In determining the amount bid by each bidder, the City shall disregard mathematical errors in <br />addition, subtraction, multiplication, and division that appear obvious on the face of the Proposal. <br />When such a mathematical error appears on the face of the Proposal, the City shall have the right <br />to correct such error and to compute the total amount bid by said bidder on the basis of the <br />corrected figure or figures. <br />When an item price is required to be set forth in the Proposal, and the total for the item set forth <br />separately does not agree with a figure which is derived by multiplying the item price times the <br />Project Coordinators' estimate of the quantity of work to be performed for said item, the item price <br />shall prevail over the sum set forth as the total for the item unless, in the sole discretion of the City <br />City of Santa Ana — Banking Services Page 28 <br />
