Each firm must include the following references, <br />Complete Client Reference Form (Attachment 5) listing five (5) client <br />references. <br />We have included 5 California Government Agencies as references as well as an <br />out of state reference. <br />Please refer to Attachment 5 for the completed Client Reference Form. <br />2. List similar services performed for all similar organizations /entities in the <br />last five years and when performed. Please include names of organizations, <br />and names and telephone numbers of persons who can be contacted with <br />regard to the services your Bank has provided. <br />As previously mentioned, J.P. Morgan's Government Banking teams currently <br />support more than 2,35o active State, County and Municipal customer <br />relationships in more than 28 states. Please refer to Attachment 5 for references <br />utilizing similar services. We are happy to provide additional references as <br />needed. <br />3. List all public agencies for which Agreements were terminated in the last <br />three years. Please include names of organizations, and names and telephone <br />numbers of persons who can be contacted.
