INSURANCE NOT REQUIRED <br />WORK MAY PROCEED <br />CLERK OF COUNCIL <br />nor:= tz(�1ae1t, <br />A-2016-295 <br />FIRST AMENDMENT TO EMERGENCY SOLUTIONS GRANT <br />SUBRECIPIENT AGREEMENT BETWEEN TIME CITY OF SANTA ANA AND <br />ILLUMINATION FOUNDATION <br />(24 CFR Parts 41 and 576) <br />THIS FIRST AMENDMENT TO THE EMERGENCY SOLUTIONS GRANT AGREEMENT is <br />entered into this 18a' day of October, 2016, by and between the City of Santa Ana, a charter city and <br />municipal corporation of the State of California ("City") and ILLUMINATION FOUNDATION, a <br />California nonprofit organization ("Subrecipioat"). <br />RECITALS <br />A. On July 22, 2016, the City entered into an Emergency Solutions Grant Subrecipiont Agreement <br />#A-2016-064 with Illumination Foundation to provide Emergency Funds from the United <br />States Department: of Housing and urban Development (HUD) to be used in the operation of an <br />emergency solution program for the homeless or at risk of homelessness of the City of. Santa <br />Ana ("said Agreement"). <br />B. In accordance with the terms and conditions of said Agreement, the parties desire to amend to <br />increase funds to the Subrecipient, which need to be spent by June 30, 2017. <br />NOW THEREFORE, in consideration of the mutual and respective promises, and subject to the <br />terms and conditions of said Agreement, except as herein modified, the parties agree as follows: <br />1. Paragraph 5, shall be amended to increase funding to Subrecipient by $45,510.55 as part of <br />its Rapid Re -Horning program to hire a Housing Navigator for homeless individuals and <br />families residing in the Santa Ana Civic Center, for a total sum not to exceed $150,510.55. <br />2. Section I, subsection A, shall be amended to add the specific tasks required of the new <br />Housing Navigator position, as detailed in the Scope of Services attached hereto as Exhibit A <br />and incorporated herein by reference. <br />3, Section III, shall be amended to increase funding to Subrecipient by $45,510.55, for a total <br />sum not to exceed $150,510.55. <br />4. Fxeept as hereinabove modified, the terms and conditions of said Agreement remain unchanged <br />and in full force and effect. <br />
