Within 90 days of the end of the period of performance, or after expiration of a close out <br />extension via GAN, recipients must submit a final FFR and final performance progress <br />report detailing all accomplishments and a qualitative summary of the impact of those <br />accomplishments throughout the period of performance, as well as all the following <br />documentation: <br />1) Final request for payment, if applicable; <br />2) SF -425 —Final Federal Financial Report; <br />3) SF -PPR — Final Performance Progress Report to include final Activity Outline <br />Report from Work Plan; <br />4) A qualitative summary of the impact of accomplishments throughout the entire <br />period of performance. The summary is submitted to the respective Regional <br />Program Manager in a Word document; <br />5) SF -428 — Tangible Personal Property Report — Inventory of all tangible personal <br />property acquired using funds from this award; and <br />6) Other documents required by program guidance or terms and conditions of the <br />award. <br />If applicable, an inventory of all construction projects that used funds from this program <br />has to be reported using the Real Property Status Report (SF -429) available at <br />htlp: / /www.whitchouse.gov /sites /default/files /omb /grants /a roved forms /0- 429.ndf. <br />Additionally, the recipient must liquidate all obligations incurred under the EMPG <br />Program award no later than 90 calendar days after the end date of the period of <br />performance or expiration of a GAN that closes out the award, whichever comes first. If <br />a recipient fails to liquidate their obligations within this time period, FEMA may disallow <br />the costs associated with those unliquidated obligations. Recipients also are responsible <br />for promptly returning to FEMA the balance of any funds that have been drawn down, <br />but remain unliquidated. <br />After final reports have been reviewed and approved by FEMA, a close -out notice will be <br />completed to close out the grant. The notice will indicate the period of performance as <br />closed, list any remaining funds the recipient has not drawn down that will be <br />deobligated, and address requirements for record retention, and disposition and reporting <br />requirements for any equipment or real property purchased using EMPG grant funding. <br />In addition, any EMPG recipient that issues subgrants to any subrecipients is responsible <br />for closing out those subgrants as described in 2 C.F.R. § 200.343. EMPG recipients <br />must ensure that they complete the closeout of their subgrants in time to submit all <br />necessary documentation and information to FEMA during the closeout of their own <br />grant award. <br />G.
