Holman Capital Corporation <br />25201 Paseo de Alicia, Suite 290 <br />Laguna Hills, CA 92653 <br />EXHIBIT C-1 <br />INSURANCE CERTIFICATION <br />June 15, 2017 <br />Re: Equipment Lease -Purchase Agreement dated June 15, 2017 <br />In connection with the above -referenced Lease Agreement, City of Santa Ana, as lessee (the <br />"Lessee ") certifies that it has instructed the insurance agent named below (please fill in name, address, and <br />telephone number): <br />Name of Agent: Big Independent Cities Excess Pool <br />Address: 1750 Creekside Oaks Drive, Suite 200, Sacramento, CA 95833 <br />Phone: 800-541-4591 <br />to issue: <br />Liability Insurance. Lessee is required to maintain public liability insurance, <br />personal injury and property damage with policy limits of $5,000,000. The policy <br />X should be endorsed to name Santa Cruz County Bank (the assignee of Holman Capital <br />Corporation) as an additional insured. <br />Casualty Insurance. Lessee is required to maintain all risk extended coverage, <br />malicious mischief and vandalism insurance for the Equipment described in the <br />X above -referenced Equipment Schedule in an amount not less than the greater of <br />$1,462.000.00 or the full replacement cost of the Equipment. Such insurance shall <br />be endorsed to name Santa Cruz County Bank as a lender loss payee with respect to <br />such Equipment. <br />The required insurance should also be endorsed to give Santa Cruz County Bank at least 30 days <br />prior written notice of the effective date of any material alteration or cancellation of coverage, and an <br />endorsement confirming that the interest of Santa Cruz County Bank shall not be invalidated by any actions, <br />inactions, breach of warranty or conditions or negligence of Lessee. <br />Proof of insurance coverage will be provided to Santa Cruz County Bank prior to and/or <br />commensurate with the Commencement Date of the Lease. <br />Very truly yours, <br />CITY OF SANTA ANA, as Lessee <br />By: it � X <br />Namts WthiaKurtz 6 <br />Title: Interim City Manager <br />C-1 <br />
