Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. <br />What specific activities will be undertaken during the contract period. Only the <br />viewable space will print. <br />Desposit only assistance will be be provided to chronically homeless program participants who are <br />entering permanent supportive housing opportunties in which on-going rental assistance is provided <br />through Proj ect Based Vouchers. All referrals for the program will come from the Coordinated Entry <br />System and will have connections to the City of Santa Ana and to the greatest extent possible, with <br />preference for those who currently reside in Civic Center area. <br />Additionally, Mercy House will provide Rapid Rehousing assistance to one additional homeless <br />household who will receive security deposit assistance, short term to medium tern rental assistance, case <br />managment and housing search and placement services. <br />Schedule of Performance <br />Estimate the number of ONLY unduplicated <br />Santa Ana participants to be served by the funded program during the 12 - <br />month contract period per quarter. <br />(Enter number of new Santa Ana Participants served each quarter.
