Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />"IF deploys basic needs support (hygiene, socks, food, rain gear) to unsheltered individuals in Santa Ana. This outreach <br />activity create opportunities for staff to informally engage and assess unsheltered individuals within their peer and <br />community setting. IF staff conducts outreach in riverbeds, parking lots, bus stops, and motels. Clients are screened for <br />eligibility using VI-SPDAT and utilizes the CoC-wide system of Coordinated Entry/Coordinated Assessment. From this <br />assessment, clients are provided with or referred to the appropriate housing services, including diversion services. <br />-Diversions are offered as a walk-in service designed as a first point of entry intervention to problem solve a housing <br />crisis in such a way that families and individuals never have a need to enter the shelter system. If needed , IF provides <br />access to programs and services such as wraparound services, emergency housing, other housing programs, and <br />homeless prevention funds. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12 -month <br />contract period per quarter.
