Receive and file rates for employee health insurance through CalPERS for calendar year <br />2018. <br />2. Receive and file rates for employee dental insurance with Delta Dental and MetLife, employee <br />life and long term disability insurance coverage through Aetna and employee vision insurance <br />through EyeMed for the calendar years 2018 and 2019. <br />3. Authorize the City Manager and Clerk of Council to execute the attached agreement with <br />Reach, subject to non -substantive changes approved by the City Manager and City Attorney, to <br />provide employee assistance plan services from January 1, 2018 to December 31, 2020 at a <br />monthly cost of $1.85 per full time employee and $1.25 per part-time employee. <br />4. Authorize the City Manager and Clerk of the Council to execute an agreement with TASC, <br />subject to non -substantive changes approved by the City Manager and City Attorney, to provide <br />employee flexible spending accounts and Retiree Dental Billing services, effective April 1, 2017. <br />5. Receive and file continued agreement with ICMA-RC as the Plan Administrator of the <br />VantageCare Retiree Health Savings Plan through December 31, 2018. <br />DISCUSSION <br />The City utilizes the services of two insurance brokers Keenan & Associates and Cook Insurance <br />Services. Keenan is the broker of record on the life, accidental death & dismemberment (AD&D) <br />25E-1 <br />
