City of Santa Ana CDBG Scope of Work <br />Program Year 2017-18 (July 1, 2017 -June 30, 2018) <br />Name of Organization The Illumination Foundation <br />Name of Funded Program Children's Resource Center (Santa Ana Only) <br />Annual Accomplishment Goal <br />Unduplicated Participants anticipated to be served during the 12 -month contract Perl2d. <br />225 TOTAL 1 225 Santa Ana Participants 100% 225 Low Income Participants 100% <br />Program and Funding Description <br />The Children's Resource Center is also piloting a Youth After -School Program for adolescents (ages 13-17) who would <br />previously have aged out of CRC services. Each adolescent, in addition to having their own time for homework help <br />and educational offerings, will have time dedicated to being mentored by a staff member and mentoring a younger <br />student in turn. This youth program will provide age-appropriate services and activities for teenagers, who are ready <br />for more autonomy and a leadership role. <br />In order to maximize program effectiveness, tutoring, homework assistance, recreation opportunities, youth <br />mentorship, and nutritious meals are offered every day from Monday through Friday, from 2:30pm - 6:O0pm, during <br />the school year. On a weekly basis, meals are also provided to the greater community. Enrichment activities are <br />offered on a weekly or monthly basis. Mobile and vision health services are offered at the Santa Ana CRC as needed. <br />Additionally, a Child and Family Case Manager will provide intensive case management services for the highly <br />vulnerable and homless children at the CRC. During school vacations, the CRC is open during the day every day from <br />Monday through Friday to provide meals and enrichment activities. <br />The Santa Ana Children's Resource Center serves the Willard neighborhood. All children served are homeless or at - <br />risk of homelessness, and 98% are Latino.
