Only the <br />viewable space will print. <br />Our Emergency Shelter Program addresses the need for year-round emergency shelter and the <br />continued need for immediate shelter and supportive services to assist families with children break the <br />cycle of homelessness. The emergency shelter program provides immediate safe shelter and <br />supportive services to homeless women and homeless mothers with children on an emergent basis. <br />At the point of program entry an assessment is completed to identify every barrier to housing stability <br />that may exist for the family, and those barriers are addressed through goal setting and accountabilities <br />under the care and guidance of our staff throughout program participation. Our core supportive <br />services include intensive case management, life skills classes, assistance with securing gainful <br />employment, assistance with budgeting and savings, and essential physical needs such as food, <br />transportation, clothing, household items, personal and infant/children items. Program length of stay is <br />dependent on each household individual circumstance. The outcome for each participant family is the <br />same: long-term stable housing. Families that are well-suited for a rapid re -housing approach are <br />assisted in securing housing working with community based organizations. Chronic homeless, high- <br />risk, and special needs families that require additional time and services within the structure of a <br />housing program may be transferred to an available unit within our Transitional Housing Program, with <br />long-term stability solutions being either placement in independent housing, one of our Permanent <br />Affordable Housing projects, or other community based Permanent Supportive Housing projects, <br />dependent on household need, financial circumstances and support systems in place, and available <br />housing resources. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12 -month <br />contract period per quarter.
