IV <br />V. <br />Congress in connection with awarding of any federal contract, the malting of any federal grant or loan, entering into <br />any cooperative agreement and the extension, renewal, amendment, or modification of any federal contract, grant, <br />loan, or cooperative agreement. The SUBRECIPIENT shall sign a certification to that effect in a form as set forth in <br />Exhibit C, attached hereto and by this reference incorporated herein. The SUBRECIPIENT shall submit said signed <br />certification to the CITY prior to performing any of its obligations under this AGREEMENT and prior to any <br />obligation arising on the part of the CITY to pay any si:ms to the SUBRECIPIENT under the terms and conditions of <br />this AGREEMENT. If any fiords other than Federal appropriated funds have been paid or will be paid to any person <br />for influencing or attempting to influence an officer or employee of any agency, a Member of Congress, an officer or <br />employee of Congress, or all employee of a Member of Congress in connection with this Federal contract, grant, loan, <br />or cooperative agreement, the undersigned shall complete and submit a. 'Disclosure Fonn to Report Lobbying," in <br />accordance with its instructions (see Exhibit D). <br />NOTICES <br />The SUBRECIPIENT and the CITY agree that all notices required by this AGREEMENT shall be made in writing <br />and delivered via mail. (postage prepaid); commercial courier; personal delivery; or sent by facsimile or otlier <br />electronic means (provided that receipt is eonfinned). Suite 106 <br />Huntington Beach, CA 92647 <br />714-596-1380 <br />The SU13UCIPIFNT must work with the Continuum of Care ("CoC") to ensure the screening, assessment, and <br />referral of program participants are consistent with the CITY's written standards for providing ESG assistance as <br />described in its consolidated plan, The SUBRECIPIEN'T must ]seep documentation evidencing the use of, and <br />written intake procedures for, the centralized or coordinated assessment system(s) developed by the CoC in <br />accordance with the requirements established by HUD.
