Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. <br />What specific activities will be undertaken during the contract period. Only the <br />viewable space will print. <br />211OC will facilitate HMIS coordination, communication, training, and technical assistance of Santa Ana ESG <br />sub -recipients to ensure adherence to data quality, supporting the City's efforts to fully comply with the <br />HEARTH Act of 2009 and continued implementation CES. 211OC will: <br />-provide user meetings, <br />-perform site visits, <br />-provide training/technical assistance, <br />-run data qulaity reports to post and make available online (www.21loc.org) <br />-complete project set-ups, and <br />-assess homeless trhough CES; link hornless to hoursing through CES <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12 -month <br />contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
