RECEIVE AND FILE EMPLOYEE HEALTH INSURANCE RATE RENEWALS <br />FOR CALENDAR YEAR 2018 AND EMPLOYEE DENTAL, LIFE AND LONG- <br />TERM DISABILITY, AND VISION INSURANCE FOR CALENDAR YEARS 2018- <br />2019; APPROVE AN AGREEMENT FOR EMPLOYEE ASSISTANCE PLAN <br />SERVICES AND FOR MANAGEMENT OF FLEX SPENDING ACCOUNTS AND <br />RETIREE DENTAL BILLING SERVICES; AND, RECEIVE AND FILE <br />CONTINUED AGREEMENT WITH ICMA-RC VANTAGECARE RETIREMENT <br />HEALTH SAVINGS (RHS) PLAN {STRATEGIC PLAN NO. 7, 61 - Personnel <br />Services Agency <br />MOTION: <br />1. Receive and file rates for employee health insurance through CaIPERS <br />for calendar year 2018. <br />2. Receive and file rates for employee dental insurance with Delta Dental <br />and MetLife, employee life and long-term disability insurance coverage <br />through Aetna and employee vision insurance through EyeMed for the <br />calendar years 2018 and 2019. <br />3. Authorize the City Manager and Clerk of Council to execute an <br />agreement with REACH to provide employee assistance plan services <br />from January 1, 2018 to December 31, 2020 at a monthly cost of $1.85 <br />per full time employee and $1.25 per part-time employee, subject to <br />non -substantive changes approved by the City Manager and City <br />Attorney (AGMT. No. 2017-241). <br />4. Authorize the City Manager and Clerk of the Council to execute an <br />agreement with TASC to provide employee flexible spending accounts, <br />subject to non -substantive changes approved by the City Manager and <br />City Attorney (AGMT. No. 2017-242). <br />5. Authorize the City Manager and Clerk of the Council to execute an <br />agreement with TASC to provide Retiree Dental Billing services, <br />effective April 1, 2017 subject to non -substantive changes approved by <br />the City Manager and City Attorney (AGMT. No. 2017-243). <br />6.
