AP -10 Consultation — 91.100, 91.200(b), 91.215(1) <br />Introduction <br />The United States Department of Housing and Urban Development (HUD) now requires that entitlement <br />jurisdictions submit the Annual Action Plan and the Five -Year Consolidated Plan through the Integrated <br />Disbursement and Information System (IDIS). The IDIS template contains relevant section and a series of <br />questions to complete the plan. The City of Santa Ana's 2017-2018 Annual Action Plan was developed <br />using the IDIS template and in accordance with statutory requirements,and federal regulations. <br />Provide a concise summary of the jurisdiction's activities to enhance coordination between <br />public and assisted housing providers and private and governmental health, mental health <br />and service agencies (91.215(1)) <br />The City of Santa Ana has it's own Housing Authority and works collaboratively on all programs. Housing <br />Authority and City staff meet quarterly with County Mental Healthi workers in addition to sitting on <br />various committees together. ESG funds will continue to fund mental health outreach,services in FY <br />2017-2018. <br />Describe coordination with the Continuum of Care and efforts to address the needs of <br />homeless persons (particularly chronically, homeless individualsand families, families with <br />children, veterans, and unaccompanied youth) and persons at risk of homelessness. <br />2-1-1 Orange County is the'County's lead with the Coordinated Entry System as well as the Homeless <br />Management Information System (HMIs): Each'sub-recipient that the City funds is required to partipate <br />in this system. With the;establishment of the Coordinated Entry System, organizations are required to <br />lower their barriers to entryand adopt,a Housing First model. Street Outreach Teams are administering <br />the Vulnerability Index - Service Prioritiiation Decision Assistance Tool (VI-SPDAT) to help in the <br />placement of housing.
