• The following materials are obtained during intake: <br />o Copies of any protective order. <br />o Current court orders, including any judicial council form related to supervised <br />visitation orders. <br />o A report of any written records of allegations of domestic violence or abuse; and <br />o An account of the child's health needs if the child has a chronic health condition. <br />Clients are given the following: <br />• Business card. <br />• Family concern form. <br />• Guidelines for preparing children for visitation. <br />• Copy of client guidelines which are signed by both of the parties. <br />Intake checklist outlining paperwork, confidentiality, expectations and fees for service. <br />Orientation to the following program start up rules: <br />Punctuality. <br />• Termination of services/security procedures. <br />Designated entrances to the building. <br />Designated times of arrival/departure. <br />Parties are sent a confirmation call outlining the designated times of arrival and a <br />reminder of necessary program information. <br />Orientation <br />Purpose: <br />The practice of orientation, conducted individually with each member of the family, will occur at <br />F.A.C.E.S., separate from and prior to the first visit or exchange in order to: <br />• Establish a purposeful relationship of engaging with each parent and age appropriate <br />child. <br />• Exchange information with each parent and child(ren) so that F.A.C.E.S. can provide <br />meaningful and safe services to each individual, and each individual can best use the <br />services offered by F.A.C.E.S. This includes identifying and responding to the complex <br />needs of each person. <br />• Begin the process of undoing the harm to child(ren) and adult victims caused by the <br />violence and reducing the opportunity and inclination for batterers to cause further harm. <br />ra <br />
