A-2017-311 <br />ititt;(r 'F10 <br />` i� DEG 14 2017 <br />0 1% k.2) FIRST AMENDMENT TO EMERGENCY SOLUTIONS GRANT <br />SURRECIVIENT AGREEMENT BETWEEN THE CITY OF SANTA ANA AND <br />ILLUMINATION FOUNDATION <br />(24 CFR farts 91 and 576) <br />THIS FIRST AMENDMENT TO THE EMERGENCY SOLUTIONS GRANT AGREEMENT is <br />entered into this —day of November, 2017, by and between the City of Santa Ana, a charter city and <br />municipal corporation of the State of California ("City"), and ILLUMINATION FOUNDATION, a <br />California nonprofit organization ("Subrecipient"). <br />"UTA <br />A. Ondune, 22, 2017, the City entered into an Emergency Solutions Grant Subrecipient Agreement <br />#A-2017-072-03 with Ilhuniraation Foundation to provide Emergency Funds from the United <br />States Department of Housing and urban Development (HUD) to be used in the operation of an <br />emergency solution program for the homeless or at risk of homelessness of the City of Santa <br />Ana C'said Agreement"). <br />B. In accordance with the terms acid conditions of said .Agreement, the parties desire to amend to <br />increase funds to the Subrecipient, which need to be spent by June 30, 2018. <br />NOW THEREFORE, in consideration of the mutual and respective promises, andsubject to the <br />teens and condition's of said Agreement, except as herein modified, the parties agree as follows: <br />I, Paragraph 5, shall be amended to increase funding to Subrecipient by $60,975,81, which <br />includes: $21,000 in additional Street outreach funds for a total of $67,000; $7,976 in now <br />Shelter funds; and, $92,000 in additional Rapid Re -housing funds for a total of $77,000. "Elie <br />new total sum shall not. exceed $207,129,81 in grant funding. <br />2. Section I, subsection A, shall be amended to add thespecific tasks required of the additional <br />funds to target and, house individuals directly from the Plaza of the Flags. The program focus <br />and goals are to stabilize individuals with wrap-around case management services, provide <br />bridge housing and/or shared housing assistance and seek pennanent housing, as detailed, in <br />the Scope of Serviccs attached hereto as Exhibit A and incorporated heroin by reference, <br />Section III, shall be amended to increase funding to Subrecipient by $60,975.81, for a total <br />sunt not to exceed $2.07,129.81. Subrecipient's new budget for these additional funds is <br />attached hereto as Exhibit tl and incorporated herein by reference, <br />4. Except as hereinabove modified, the terms and conditions of said Agreement remain unchanged <br />and in full force aid effect. <br />111 <br />
