Description of Work - In the space below, describe the program to be funded during the 12 -month contract <br />period. What specific activities will be undertaken during the contract period. Only the viewable space will print. <br />Illumination Foundation, in partnership with the Santa Ana Police Department's HEART Team, will target up <br />to 20 homeless individuals who currently reside in the City of Santa Ana's Plaza of The Flags for engagement <br />in supportive services and possible placement into Rapid Rehousing. With the support of the City of Santa <br />Ana ESG funds, Illumination will aim to house clients in Micro -Community shared housing environments with <br />the goal on long term housing stablization. <br />Schedule of Performance <br />Estimate the number of ONLY unduplicated Santa Ana participants to be served by the funded program during <br />the 12 -month contract period per quarter.
