Please include this page on top of the response. <br />Attachment 3-8 — Cost of Services: The Santa Ana Public Library requires a collection that meets the needs of <br />its diverse population ranging from various socio-economic demographics. Additionally, a collection that <br />reflects the community's dominant languages is ideal and that is accessible on various platforms for optimal <br />use. <br />Certification • I certify that i have read, understand and agree to the terms and conditions of this Request for <br />Proposals. I have examined the Scope of Services (Exhibit A) and am familiar with the scope of services. I <br />am familiar with all the existing conditions and limitation that may impact services. I understand and agree that <br />I am responsible for reporting any errors, omissions or discrepancies to the City for clarification prior to the <br />submission of my proposal. <br />The Cost Proposal is to include the following information: <br />1) A complete, Itemized proposed cost list for each requested service as described in the Scope of Services <br />(Exhibit A). <br />2) The itemized breakdown must show estimated man hours rates and totals for each service. <br />3) Any equipment will be purchased by the contractor only after discussed and authorized by the City Project <br />Manager or designee in writing. <br />City of Santa Ana RFP 17-141 <br />Page 27 <br />25H-26 <br />
