INFORMATION REQUESTED FROM PROPOSER <br />To achieve a uniform review process and obtain the maximum degree of comparability, it is <br />required that proposals be organized in the manner specified below: <br />1. Title <br />Show the RFP subject, the name of proposer firm, local address, telephone number, name <br />of contact person and date. <br />2. Letter of Transmittal <br />Briefly state the proposer's understanding of the work to be done and make a positive <br />commitment to perform the work within the specified time period. <br />Give the names of the persons who will be authorized to make representations for the <br />proposer, their titles, addresses, e-mail addresses, telephone numbers, and fax numbers. <br />3. Profile and Qualifications of the Proposer <br />Identify personnel who will provide the day to day administration of the CDBG program <br />including their resumes. <br />Describe recent CDBG experience similar to the type requested. <br />4. References <br />Give names, addresses, and telephone numbers of three (3) client contacts for such audits. <br />25J-18 EXHIBIT 1 <br />
