This amendment is between the City of Anaheim and the Grant Sub -Recipient:
Santa Ana Police Department
The Sub -Recipient agrees to the amendment of the Grant Sub -Recipient Award Agreement as specified
below:
Extend FY2017 City of Anaheim sub -recipient performance period end date from 8/15/2020 to
9/30/2021 for all Human Trafficking Grant projects.
The last grant claim must be received by the City of Anaheim by no later than 10/15/2021.
Please refer to the FY17 Human Trafficking Enhanced Collaborative Model Program Sub -Recipient Grant
Guide for all reporting and reimbursement deadlines associated with the sub -award agreement.
