The formation of this team brings all stakeholders together in the field to efficiently address issues. –QOLT staff in the field provides outreach, including shelter and mental health services, to those experiencing homelessness. •City Net Homeless Outreach Team –Outreach to unsheltered individuals to offer support and to connect with critical services.
